Caring for Childhood Chronic Conditions

The CCCI team partners with pediatric specialty clinics at the 91��Ƶ’s Children’s Hospital to improve care for Medicaid-eligible children with chronic conditions. We collaborate with VCHIP’s Health Services Research Team to analyze data and identify improvement opportunities.

The inaugural Care Coordination Summit (PDF) brought together numerous interdisciplinary partners and family health partners around improving care coordination services for Children and Youth with Special Health Needs. Summit participants developed priority areas for improving and uniting care coordination efforts for children and youth with special health needs.
Publication highlights VCHIP’s work studying children with medical issues and mental disorders and the effects of the pandemic.
